I/we hereby declared that all the information given to NHRA is true and not fake. NHRA can use all the information and documents to procced the application. NHRA will have the right to save all the information including communications. If any document seems or found to be fake or duplicate that application will canceled and the fee will be forfit.
I/we also declare that we are the Importers/Trader/Manufacturer. We guarantee that this portal will be utilized only by applicant for Authorized Representative Registration (AR) and/or Medical Device Registration and credentials will not be shared with third party. We also acknowledge that the credentials cannot be used by consultant without informing NHRA. Consultant must be having the approval from NHRA for providing such services.
